Overview

The Clinical Nurse Navigation is responsible for coordinating and collecting data and, works with patients to ensure their health care needs and barriers to care are addressed in a timely fashion. Serves as advocate and medical contact person for patient, family, healthcare team members, employees and outside agencies to assure continuity of care and optimal clinical and resource outcomes. May periodically assist with triaging patients.

Responsibilities

  • Provider clinical support and decision making to the patient navigation centers’ patient service representatives
  • Act as a liaison between the patient navigation center and the health centers
  • Direct patient call flow to ensure patients disposition is accurate 
  • Provide medical telephonic coaching, information and educational materials and resources to patients as needed. 
  • Assist patients with securing appropriate medical care and services within the health system and community and facilitate patient access to medical home providers, staff and resources. 
  • Link patients to appropriate resources, services and support for continuation and coordination/navigation of care. 
  • Assist in determining the health status and needs of patients based on interpretation of health related data and preventative health practices. 
  • Accurately and promptly schedule, reschedule, and cancel physician and non-physician patient appointments.
  • Identify urgent scheduling needs and prioritize them with the appropriate health center. 
  • Identify urgent and potentially urgent patient care situations and escalate as appropriate for a positive patient outcome 
  • Perform other duties as assigned. 
  • Coordinate preventative, wellness and chronic needs management services using evidenced based 

Knowledge/Skills/Abilities:

  • High level of skill at building relationships and providing excellent customer service. 
  • High degree of knowledge and competency in the practice of medicine and associated charting requirements. 
  • High level of problem solving skill to better serve patients and staff. 
  • Strong attention to detail and accuracy. 
  • Excellent verbal and written communication skills. 
  • Demonstrated experience with the delivery of quality nursing practices and duties assigned to this role. 
  • Demonstrated experience developing personal practice guidelines that are dependent on knowing what the aspects of the situation look and sound like in actual patient situations. 
  • Demonstrated experience in managing multiple nursing activities at once. 
  • Demonstrated familiarity with tools, technology, and systems typically found within most health care environments (i.e. personal computer skills, spreadsheets, word processing, patient records systems, EMR systems, etc.). 

Qualifications

MINIMUM EDUCATION:  H.S. diploma or equivalent and graduation from an accredited school of nursing.

 

PREFERRED EDUCATION:  Bachelor of Science in Nursing (BSN) preferred

 

MINIMUM EXPERIENCE:  1-2 years of nursing experience preferred. Six months of nursing experience with at least 5 years’ experience as a Medical Assistant and/or Care Coordinator in an outpatient medical setting may be accepted in lieu of.

REQUIRED CERTIFICATIONS/LICENSURE:

 

1.  Current LVN or Registered Nurse License for the State of Texas.
2.  Current healthcare provider Cardiopulmonary Resuscitation (CPR) course completion card and will be required to maintain a current CPR certification during employment.
