Overview

In collaboration with patients, families (as defined by the patient) and staff across all disciplines and departments is responsible for providing drug therapy management under protocols approved by the CommUnityCare medical staff.

Responsibilities

  • Identify and resolve medication-related problems.
  • Manage chronic diseases as outlined in the Clinical Pharmacy protocols (i.e. anticoagulation, asthma, chronic obstructive pulmonary disease, congestive heart failure, diabetes, polypharmacy).
  • Assist with medication refills as outlined in the Clinical Pharmacy protocols.
  • Assist with adjudication of non-formulary drug requests and act as a clinic resource regarding the formulary.
  • Assume an active role in the education of pharmacy students and residents.
  • Provide patient education regarding medications and chronic disease states.
  • Serve as drug information resource for medical staff.
  • May be responsible for educational instruction or instructional team participation for the DMST program.
  • Perform other duties as assigned.

  • Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare Standard Operating Procedures and CommUnityCare Policies and Procedures.
  • Develop and maintain favorable internal relationships, partnerships with co-workers.
  • Interact respectfully and collaboratively with patients and their families, striving to develop favorable relationships with families.
  • Collaborate with all members of the care team in providing patient-centered care.
  • Ensure all actions, job performance, personal conduct and communications represent CommUnityCare in a highly professional manner at all times.
  • Uphold and ensure compliance and attention to all company policies and procedures as well as the overall mission and values of the organization.
  • Complete all mandatory training and maintain appropriate credentials/licensure.

Qualifications

  • High level of skill at building relationships and providing excellent customer service.

  • High degree of knowledge of drug therapy and dispensing medications.
  • Basic computer systems knowledge, including but not limited to Microsoft Office products.
  • Strong attention to detail and accuracy.
  • Excellent verbal and written communication skills.
  • Ability to utilize drug information resources and communicate results written/verbally.
  • Bilingual (English/Spanish) strongly preferred.

Education:

  • Doctor of Pharmacy.
  • 1 year post-doctoral residency (2 year post-doctoral residency preferred).
  • Board certification preferred.

Knowledge and Experience:

  • 3-5 years of clinical experience will be considered in lieu of post-doctoral residency.
  • Demonstrated knowledge of local, state, and federal laws pertaining to Clinical Pharmacy activities as related to the use of drugs and other substances.
  • Knowledge of drug distribution chain and general trends in the pharmaceutical marketplace.
  • Demonstrated knowledge of chronic disease state guidelines.
  • Past preceptorship of students/residents preferred.
