The Clinical Screener is responsible for ensuring incoming call flow management to the nursing services team at the Patient Navigation Center.


  • Coordinates and collect data and work with patients to ensure their health care needs and barriers to care are addressed in a timely fashion.
  • Serves as advocate and medical contact person for patient, family, healthcare team members, CUC team members and outside agencies to assure continuity of care and optimal.
  • Prioritize incoming patient and healthcare agency calls and direct call flow to ensure patient disposition is accurate and appropriate.
  • Assist in determining the health status and needs of patients based on interpretation of health related data and preventative health practices.
  • Make rapid decisions related to incoming calls.
  • Identify urgent scheduling needs and prioritize them with the Patient Navigation Center team.
  • Identify urgent and potentially urgent patient care situations and escalate as appropriate for a positive patient outcome.
  • Act as liaison with the Patient Navigation Center.
  • Provide medical telephonic coaching, information and educational materials and resources to patients as needed.
  • Assist patients with securing appropriate medical care and services within the health system and community and facilitate patient access to medical home providers, staff and resources.
  • Link patients to appropriate resources, services and support for continuation and coordination/navigation of care.
  • Accurately and promptly schedule, reschedule and cancel physician and non-physician patient appointments.
  • Perform other duties as assigned



  • Self-directed and able to manage multiple projects.
  • Ability to engage patient in education and self- management.
  • Strong verbal and written communication skills.
  • Proficient in patient triage and navigation of medical records information.
  • Demonstrated knowledge of the four steps in systematic process, which include assessment, planning, implementation and evaluation.
  • Knowledge of common medical diagnosis, drug and other therapies and treatments.
  • Demonstrated success in obtaining patient history using a structured assessment tool.
  • Demonstrated ability to identify common actual and potential health care needs of patients.
  • Demonstrated ability to administer medications and treatments and perform procedures safely.
  • Demonstrated experience with direct patient services and the proper knowledge and use of medical devices and equipment.
  • Demonstrated familiarity with tools, technology and systems typically found within most health care environments (i.e. personal computer skills, spreadsheets, word processing, patient record systems, EMR systems, etc.).


MINIMUM EDUCATION:  High School Diploma or equivalent


MINIMUM EXPERIENCE:  1-2 years experience as a Medical Assistant or providing direct patient care


REQUIRED CERTIFICATIONS/LICENSURE:  Medical Assistant Diploma and/or National Medical Assistant Certification verifying graduation from a Medical Assistant Program or equivalent military healthcare experience.


PREFERRED CERTIFICATIONS/LICENSURE:  Current healthcare provider Cardiopulmonary Resuscitation (CPR) course completion card.  Will be required to maintain a current CPR certification during employment.
