  • Overview

    Reporting to the Chief Operating Officer (COO), the Director of Population Health is responsible for the oversight, direction, provision and quality of all Nursing and clinical and support services provided to patients of CommunityCare (CUC).  Will serve as an adviser to the COO and Chief Medical Officer(CMO) on all matters pertaining to population health lead initiatives.  Responsible for demonstrating leadership, experience, and creative approaches to the management of complex patient care, population health and management supporting the organizational and administration of healthcare delivery system.  Responsible for the management principles supporting database management, data analysis, data analytics, data display and use of statistical tools to enhance the interpretation and value of available data.  Works collaboratively with Peers, Providers, and Practice Leaders supporting all health center and administrative sites

    Responsibilities

    • Oversees the standard and practices for the organization’s nursing and clinical team services. Works with the COO and CMO in the development of patient centered care programs, policies and procedures to meet requirement for all regulatory bodies. 
    • Implements and maintains an effective and ongoing program to measure, monitor and oversees and improves the quality and appropriateness of nursing and clinical team care, treatment and services delivered to patients.
    • Provides oversight and interpretation of concurrent record review as well as quality and performance improvement activities that evaluate the effectiveness of nursing and clinical staff, including nursing peer review.
    • Provides oversight of processes to ensure application of analytics to apply evidence-based practice initiatives to achieve best practices/benchmarking.
    • Serves as a proactive liaison to the CMO to facilitate data engagement by the expanded care teams. 
    • Implements programs supporting clinical interpretation of data and interventions into healthcare practice.
    • Leads the Care Management programs, Care Coordination programs and ACO/MCO
    • Collaborates with other health care organizations, including providers and payers, to provide comprehensive, integrated services, which may include organizing care management for patient populations.
    • Implements and maintains an effective and ongoing program to measure, monitor and assess, and improve the quality and appropriateness of care management, treatment, and services delivered to
    • Ensures performance goals and metrics are developed and met.
    • Supervise and direct teams that oversee and interface with the patient, family and primary care provider to ensure that the patient complies with the provider’s treatment pl
    • Direct the Care Management team’s coordination with internal and external social services agencies and programs to ensure that patient receives needed services.
    • Collaborates with other health care organizations, including providers and payers, to provide comprehensive, integrated services, which may include organizing care management for patient
    • Anticipates potential problem situations to the best of ability and intervenes to offset any adverse impact. Analyzes and works collectively to bring emerging problems in the clinical delivery of services to the COO as appropriate.

    • High degree of knowledge and competency in the practice of nursing.

    People Management/Department Management/Business Unit Management:

    • Builds and maintains a superior departmental support team serving all employees in a manner that is consistent with the company’s Core Beliefs
    • Selects personnel for hire and promotion; takes appropriate actions regarding counseling, disciplinary actions, demotion and termination
    • Directs, supports and coaches direct reports
    • Assesses learning needs, develops competency plans and provides opportunities for learning
    • Participate in quality strategies to evaluate compliance with evidence-based guidelines, standards and to identify opportunities to improve patient outcomes.
    • Responsible for taking appropriate disciplinary action with nursing team members when necessary.
    • Completes the performance evaluations of care management/ care coordination staff in collaboration with site level and clinical leadership.
    • Participates in departmental and interdisciplinary committees, which influence or determine policies affecting population health practices.
    • Develop and maintain favorable internal relationships, partnerships with co-workers, including clinical managers, clinical support staff, providers and business office staff.
    • Plan and implement identified CUC strategies and goals, ensuring that his/her allotted department is working in accordance with such goals.
    • Collaborate with all members of the care team in providing patient-centered care.

    Qualifications

    MINIMUM EDUCATION:    Graduation from an accredited school of nursing.  Bachelor’s degree in nursing or related field.

     

    PREFERRED EDUCATION:   Master’s Degree in Business or Public Health is preferred.

     

    MINIMUM EXPERIENCE:

    • Five (5) to ten (10) years leadership experience in a related health care field.
    • Three (3) years experience in a supervisory role.
    • Current knowledge of state and federal nursing regulations.
    • Knowledge of health and human services environment trends and changes
    • Knowledge of local, state, and federal regulations and laws governing health and human services activities
    • Experience with statistical software, Excel and PowerPoint
    • Management in an electronic health record environment

    REQUIRED CERTIFICATIONS/LICENSURE: Active and unrestricted State of Texas Registered Nurse Liceense

     

    REQUIRED COURSES/COMPLETIONS: Basic Life Support (BLS) - Obtained through approved American Heart Association.
