Overview

Reporting to the Nurse Manager, Patient Navigation Center (PNC), the Nurse Supervisor, PNC works in collaboration with patients, families (as defined by the patient) and staff across all disciplines and departments is responsible for the development, direction, operation and performance of the PNC Clinical Team. This position will serve as an advisor to the Nurse Manager, PNC to ensure efficient, fiscally responsible and customer-responsive center operations.

Responsibilities

Primary Accountabilities:
 Supervises the work of PNC Clinical Team, including the monitoring of staff competencies, hiring of new staff, orientation of new assignments, developing goals and objectives, conducting annual performance appraisal/merit consideration and the utilization of performance resolution process when needed.
 Ensure that all staff are trained to effectively perform their required responsibilities and provided with an overview of the PNC performance goals and objectives through regular communication. Provides team members additional training if needed. Effectively organizes training materials for new employees.
 Ensures efficient and effective scheduling for the PNC Clinical Team and maintains Time & Attendance program; coordinating timekeeping and time-off requests; maintains appropriate staffing at all times.
 Performs quality assessments on Clinical Team work and recorded telephone calls.
 Identifies new protocol opportunities and recognizes (and revises) when current protocols necessitate revisions.
 Participates in process and procedures development. Works collaboratively with the PNC leadership team to improve PNC services, offer additional PNC services and achieve PNC goals.
 Oversees tasks that optimize workflow; provides suggestions to continually improve processes and the customer experience.
 Manages escalated complex patient calls.
 Fills in as Triage Nurse, when needed.
 Provides telephone triage to patients seeking advice and medical services, advises, and instructs patients on proper health care after assessing the urgency of symptoms or complaints as per protocols.
 Provides direct nursing care to patients and distributes medications and treatments according to accepted standards of care.
 Completes order corrections on diagnostics and referrals within scope.
 Teaches and counsels patients and families concerning health assessment and care including prevention of disease, disability, hygiene and health maintenance.
 Participates in the identification of patient needs for referral to resources that facilitate the continuity of care.
 Accurately and completely reports and documents status, care rendered, response to care, provider orders, contacts with patient and/or other healthcare providers.
 Accurately and completely performs patient follow-up activities including abnormal lab and diagnostics, referrals and callbacks.
 Perform other duties as assigned.

 

Knowledge/Skills/Abilities:
 Knowledge of all activities associated with the delivery of quality nursing practices and duties assigned to this role.
 Maintain knowledge of current regulatory requirements for The Joint Commission and other state and federal programs.
 High level of skill at building relationships and providing excellent customer service.
 High level of skill in managing multiple nursing activities at once.
 High level of problem solving skill to better serve patients and staff.
 Skill in medical record review and data abstraction.
 Skill in use of computer software in order to enter data, review for accuracy, and create reports.
 Strong attention to detail and accuracy.
 Excellent verbal and written communication skills.
 Demonstrated knowledge of medical record review process and reporting.
 Extensive knowledge in quality improvement/assurance processes, procedures and guidelines.
 Extensive knowledge of current evidence‐based clinical treatment guidelines.

 

People Management/Department Management/Business Unit Management:
 Supervise and direct the work activities of the PNC Clinical Team.
 Oversee the daily activities of the staff to ensure Clinical Team services meet department standards.
 Coordinate the development of and oversee the implementation of program goals and objectives, working in collaboration with the Nurse Manager, PNC; develop and approve schedules and standards for meeting program goals.
 Train staff in performance of job duties, assign job tasks, and monitor task completion.
 Review staff work for accuracy and completeness, evaluate job performance and counsel employees as needed.
 Compile data/information for reports

Qualifications

MINIMUM EDUCATION: Graduation from an accredited school of nursing.
PREFERRED EDUCATION: Bachelor of Science in Nursing (BSN) preferred
MINIMUM EXPERIENCE:
 3 years’ in nursing.
 1-year prior experience supervising and/or directing the work of others.
PREFERRED EXPERIENCE:
 3 or more years in nursing with prior experience supervising and/or directing the work of others.
REQUIRED CERTIFICATIONS/LICENSURE:
1. Current Registered Nurse License for the State of Texas.
2. Current healthcare provider Cardiopulmonary Resuscitation (CPR) course completion card and will be required to maintain a current CPR certification during employment
