Overview

Responsible for the diagnosing, treatment, and management of movement dysfunctions. Treatments plans support enhancing physical and functional abilities.

Responsibilities

Essential Duties (at least 5 that are non‐negotiable duties and are absolutely pertinent to successfully completing the job without accommodations):


 Perform initial and ongoing physical therapy evaluations according to evidence based industry standards of practice as well as state standards.
 Develop Plan of Treatment for each patient based on initial and ongoing evaluations including short and long term goals, collaboration demands of other staff members, and required patient and caregiver involvement.
 Implement Plan of Treatment on a consistent schedule and adjust this schedule and plan according to patient progress.
 Document patient progress according to approved medical documentation standards.
 Evaluate mobility, strength and current level of patient function, while setting goals to improve in these areas that are scaled to patient abilities and realistically attainable.
 Report patient progress to patient, patient caregivers and loved ones, facility staff and healthcare professionals involved in ongoing treatment/care of patients.
 Develop a plan for patients following completion of their Plan of Treatment with appropriate in‐home care services.
 Maintain professional and technical knowledge through attending educational workshops; reviewing professional publications, etc.
 Assist other facility staff members as needed to provide care and create a clean, orderly and effective environment for rehabilitation and physical therapy.
 Other duties as assigned.


Knowledge/Skills/Abilities:


 Excellent ability of matching patient needs to treatment plans and proven record of implementing these plans safely nd effectively.
 Strong ability of administering a wide range of physical therapy techniques not limited to: giving massages; applying physical agents; initiating traction; utilizing hydrotherapy tanks and whirlpool baths, ultraviolet / infrared lamps, and ultrasound machines.
 Excellent skills of assisting and directing treatments given by aides, technicians, and assistants.
 Knowledge of responsible use of facility equipment, best practices for equipment use and preventative maintenance procedures.
 Ability to implement, and evaluate operational and administrative processes.
 Experience/knowledge of Joint Commission accreditation process and requirements, as well as all federal, state and local regulations and standards associated with the delivery of care in a Federally Qualified Health Center.
 Demonstrated understanding and/or experience working in a patient‐centered medical home (PCMH) environment.
 Demonstrated understanding and/or experience working in an integrated delivery environment.
 Directly affect improvements in patient well‐being through accurate and timely diagnosis and delivery of relevant, high quality medical care.
 Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare Standard Operating Procedures and CommUnityCare Policies and Procedures.
 Interact respectfully and collaboratively with patients and their families, striving to develop favorable relationships with families.
 Collaborate with all members of the care team in providing patient‐centered care.
 Ensure all actions, job performance, personal conduct and communications represent CommUnityCare in a highly professional manner at all times.


People Management/Department Management/Business Unit Management: 


 Assesses learning needs, develops competency plans and provides opportunities for learning.

 Participate in quality strategies to evaluate compliance with evidence‐based guidelines, standards and to identify opportunities to improve patient outcomes.
 Participates in departmental and interdisciplinary committees, which influence or determine policies affecting population health practices.
 Develop and maintain favorable internal relationships, partnerships with co‐workers, including clinical managers, clinical support staff, providers and business office staff.
 Plan and implement identified CUC strategies and goals, ensuring that his/her allotted department is working in accordance with such goals.
 Collaborate with all members of the care team in providing patient‐centered care.

Qualifications

MINIMUM EDUCATION:
Completed a Doctor of Physical Therapy (DPT) degree from a school accredited by the Commission on Accreditation of Physical Therapy Education (CAPTE), unless otherwise  randfathered.

REQUIRED CERTIFICATIONS/LICENSURE:
Active and unrestricted license to practice as a physical therapist in the State of Texas

REQUIRED COURSES/COMPLETIONS (e.g., CPR)
Current Health Care Provider Cardiopulmonary Resuscitation (CPR) certification through American Heart Association or American Red Cross. Will be required to maintain a current CPR certification during employment. Current Basic Life Support Certification for Healthcare Providers through American Heart Association.
