In collaboration with patients, families (as defined by the patient) and staff across all disciplines and departments, the clinical pharmacist is responsible for providing comprehensive drug therapy management under a collaborative practice agreement approved by CommUnityCare. In partnership with the University of Texas College of Pharmacy (UTCOP) this co-funded position will have time allocated weekly dedicated to an academic faculty appointment and experiential education duties in addition to clinical responsibilities. The clinical pharmacist serves as an integral part of the multidisciplinary team and serves as the patient’s advocate in addressing their pharmaceutical care needs. This position will serve as a fundamental team member to continue to cultivate CommUnityCare’s team-based care model initiatives.
ESSENTIAL CLINICAL DUTIES
• Manage the operation of an ambulatory care clinical pharmacy service
• Provide comprehensive medication management to ambulatory care patients as outlined in the Clinical Pharmacy protocols/collaborative practice agreement
• Identify and resolve medication-related problems
• Assist with medication refills as outlined in the Clinical Pharmacy protocols/collaborative practice agreement
• Provide health improvement, wellness, and medication education to patients
• Assume an active role in the education of students, residents, or other health care professionals
• Serve as drug information resource for the health care teams 
• Assist with adjudication of non-formulary drug requests and act as a clinic resource regarding the formulary
• Work within the scope of credentialing privileges
• Collaborate and communicate with all members of the care team in providing patient-centered care
• Empower patients and/or families to be an engaged and active participant in their care 
• Educate/train Medication Refill specialists, Clinical Pharmacist technicians’ or Clinical Pharmacist trainees when needed
• Perform other duties as assigned
Academic Affiliate Responsibilities:
• Perform academic, experiential, and educational non-patient care activities at the University of Texas at Austin College of Pharmacy (UTCOP) as assigned. Will receive faculty appointment at UTCOP.
MINIMUM EDUCATION: 
• Doctor of Pharmacy
• One-year post-doctoral residency
PREFERRED EDUCATION: 
• Two-year post-doctoral residency
MINIMUM EXPERIENCE:
• Clinical experience in providing drug therapy management under a collaborative practice agreement. Five years of this experience will be considered in lieu of post-doctoral residency.
PREFERRED EXPERIENCE:
• Past preceptorship of pharmacy learners (Ex: interns, residents) 
• Basic Spanish communication skills
REQUIRED CERTIFICATIONS/LICENSURE:
• Pharmacist license with the Texas State Board of Pharmacy
• BLS/ACLS within 3 months of hire
• Board certification through the Board of Pharmacy Specialties within two years of hire
• Completion of a Postgraduate year two (PGY-1) Pharmacy Practice Residency. Five years of pharmacy experience as described above will be considered in lieu of residency.
• Clinical Pharmacy Residency Program Director: Board certification of Ambulatory Care (BCACP) required at hire
PREFERRED CERTIFICATIONS/LICENSURE:
• Completion of a Postgraduate year two (PGY-2) Ambulatory Care Residency REQUIRED COURSES/COMPLETIONS (e.g., CPR)
• Basic Life Support (BLS) - Obtained through approved American Heart Association Training Network or the Military Training Network
