Overview

Job Summary

In collaboration with teams across disciplines and departments, the Diagnostic Specialist is responsible for the identification and coordination of care for established and non-established patients, ensuring that orders are complete, results are received and providers are notified. This position assists the Diagnostic Lead and provider in ensuring that diagnostic orders are completed, and results are received by providers within the health system and community

Responsibilities

Essential Functions

  • Manage the diagnostic work queue; ensuring diagnostic reports are received by the ordering provider.
  • Scan diagnostic report into Electronic Health Records, closing orders.
  • Utilization of reports and data to coordinate continuity of care for established patients to ensure diagnostics are completed
  • Gather and analyze data from various sources to identify trends, anomalies, and underlying issues impacting processes.
  • Responsible for reporting information, assist with monitoring and maintaining the program's database to reflect productivity, benchmark data, and other indices as specified through awarded grants and programs
  • Perform standard delegation orders as needed
  • Monitor progress completion of the diagnostic reports, providing feedback to the team as needed.
  • Assist lead in questionable reports, distinguish normal and abnormal results.
  • Recommend process improvements, implement maintenance routines, or optimize systems to enhance performance and reduce the likelihood of recurring issues.
  • Operational Excellence
  • Ensure all tasks provided and associated with patient care, patient administrative processes and related duties complies with all regulatory and accreditation standards including The Joint Commission and CommUnityCare
  • Standard Operating Procedures and CommUnityCare Policies and Procedures.
  • Complete all direct and indirect care documentation in a timely manner and ensure all patient records are current and complete.
  • Attend program and/or clinic site meetings regularly. These may be related to quality assurance, patient care and other related topics.
  • Maintain all licensure and/or certificate requirements for continuing education and best practices.
  • Participate in quality improvement strategies with evidence-based guidelines and standards to identify opportunities to improve patient outcomes.
  • Work closely with medical leadership and analytics to ensure any trends are noted.
  • Develops and maintains favorable internal relationships with co-workers, including clinic managers, nursing staff, physicians and administrative staff. Upholding team-based care model and organization’score values.
  • Recommend and help implement process improvements, best practices, and preventative measures.
  • Other duties as assigned

Qualifications

Education

  • High School Diploma or equivalent (higher degree accepted)
  • Licensed Vocational Nurse Diploma

Work Experience

  • 2 years of experience as a Licensed Vocational Nurse in a clinical or ambulatory setting - Required
  • 4 years of experience as a Licensed Vocational Nurse in a clinical or ambulatory setting - Preferred
