Overview

The Permanent Supportive Housing Health Care Collaborative (PSH HCC) is a program in partnership with Integral Care that provides integrated, wraparound medical and behavioral health care to individuals transitioning out of homelessness living in permanent supportive housing at various sites throughout Travis County.
In collaboration with the Operations Administrator, the Program Manager serves as an organizational lead for the PSH HCC program, responsible for the overall development, implementation, coordination, oversight, and strategic direction of the clinical care services. The role draws on a clinical background to inform program design, staff guidance, and quality improvement. The Program Manager will collaborate to support integrated care services in partnership with the Integral Case PSH HCC Program Manager and Central Health PSH HCC Program Manager.

Responsibilities

Essential Functions:

 

Analyze population health and program performance data to improve clinical workflows, patient outcomes, and
cost effectiveness.
Lead the development of key performance metrics, program deliverables, outcome reporting, and program
performance monitoring
Support implementation, oversight, and continuous improvement of the PSH program.
Coordinate program activities and resources with internal teams and external partners to ensure effective
execution
Prepare and contribute to program reports, outcomes, and data summaries for leadership and stakeholders.
Assist with budget monitoring and tracking of expenditures to ensure alignment with program requirements and
funding guidelines.
Apply trauma-informed, culturally responsive, and harm reduction principles to program design, staff guidance,
and quality improvement initiatives.
Represent the program in organizational and community settings, sharing relevant data and insights.
Provide programmatic guidance and support with onboarding, training, coaching, huddles, workflow alignment,
and performance feedback in collaboration with clinical and operational leadership
Develop and implement patient-centered workflows, best practices, and program initiatives using programmatic
expertise.
Build and maintain collaborative relationships with internal and external stakeholders.
Contribute to program strategy, growth initiatives, and organizational representation.
Identify program risks and performance gaps through data and observation; recommend and implement solutions.
Collaborate with the care management team to support care coordination needs to resources that support
continuity of care
Lead care team huddles and staff meetings to support communication, quality, and clinical alignment.
Monitor and evaluate patient and clinical team experience as related to direct day-to-day operations of the PSH
program
Ensure PSH program complies with regulatory, accreditation, and organizational standards, policies, procedures
and government regulations.
Engage with patients, as appropriate, in a non-clinical program support capacity using trauma-informed principles
to foster trust and supportive relationships
Maintain required licensure, certifications, and professional development.
Provide oversight with quality assurance activities, chart audits, and adherence to care protocols.
Ensure staff are trained and follow emergency response and behavioral health crisis protocols. Provide guidance
during emergency and crisis situations.
Serve as operational site lead in the absence of designated leadership.
Perform other duties as assigned

requirements and guidelines. 
Ability to work under pressure, prioritize tasks, manage competing objectives in an ambitious fastpaced
environment to achieve results and meet deadlines. 
Excellent verbal and written communication skills with ability to communicate effectively with various
audiences. 
Ability to work independently and in a team environment, with demonstrated success in directing
staff to achieve a common goal. 
High level of critical thinking/problem solving skills; identifying problems, evaluating alternatives and
recommending/implementing solutions. 
Ensure all actions, work performance, personal conduct and communications always represent
CommUnityCare in a highly professional manner. 
Knowledge of commnity resoruces and programs relevant to underserved populations

 

 

 

Qualifications

QUALIFICATIONS:

 

Education:

 

Bachelor's Degree (higher degree accepted) Nursing, Social Work, Healthcare Administration, Public Health or related healthcare field -Required

 

Work Experience:

 

3 years Demonstrated work experience managing clinical projects, clinical operations and/or working in a medical office in a managerial role -Required

1 year Community-based settings -Required

2 years Demonstrated experience working with marginalized populations, including individuals experiencing homelessness, justice involvement, severe mental illness, and substance use disorders. -Required

2 years Demonstrated experience and proficiency with tools, technology and systems typically found in a healthcare environment (i.e. Microsoft Office Suite, patient record systems, EMR systems, etc.). -Required

 

2 years program development, implementation or evaluation in a community health or behavioral health setting -Preferred

2 years working with individuals experiencing homelessness -Preferred

 

Licenses and Certifications:

 

Basic Life Support Certification current for Healthcare Provider from the American Heart Association Upon Hire -Required

 

RN, LCSW or LMSW Active, Unrestricted clinical licensure in the state of Texas Upon Hire -Preferred
