Overview

In collaboration with patients, families and staff across disciplines and departments, the Diagnostic Analyst is responsible for identification and coordination of care for established and non-established patients ensuring that orders are complete, results received and provider notified. This position assists the provider in ensuring that diagnostic orders are completed, and results received by provider within the health system and community.

Responsibilities

Essential Duties (at least 5 that are non-negotiable duties and are absolutely pertinent to successfully completing the job without accommodations):


Primary Accountabilities:
• Manage the diagnostic work queue, ensuring diagnostic reports are received by the ordering provider.
• Scan diagnostic report into electronic health records, closing orders.
• Provide education to patients on diagnostic options using provided resources and materials in a culturally sensitive manner.
• Schedule diagnostic appointment with appropriate organization in alignment with patient’s healthcare goals. care.
• Utilization of reports and data to coordinate continuity of care for established patients to ensure diagnostics are completed
• Responsible for reporting information, assist with monitoring and maintaining the program's database to reflect productivity, benchmark data, and other indices as specified through awarded grants and programs. 


Operational Excellence
• Ensure all tasks provided and associated with patient care, patient administrative processes and related duties complies with all regulatory and accreditation standards including The Joint Commission and CommUnityCare Standard Operating Procedures and        CommUnityCare Policies and Procedures.
• Assist and connect patients with a resources as needed for diagnostics.
• Complete all direct and indirect care documentation in a timely manner and ensure all patient records are current and complete.
• Attend program and/or clinic site meetings regularly. These may be related to quality assurance, patient care and other related topics.
• Maintain all licensure and/or certificate requirements for continuing education and best practices.
• Participate in quality improvement strategies with evidence-based guidelines and standards to identify opportunities to improve patient outcomes.
• Support families to make informed decisions about diagnostics.
• Work closely with medical leadership and analytics to ensure any trends are noted.
• Develops and maintains favorable internal relationships with co-workers, including clinic managers, nursing staff, physicians and administrative staff. Upholding team-based care model and organization’s core values.
• Interacts respectfully and collaboratively with patients and their families, developing favorable relationship with all patients.
• Other duties as assigned


Knowledge/Skills/Abilities:
• Read, write, and maintain patient records and related administrative documentation.
• Knowledge and ability to use requisite tools, systems, technology and equipment in the collection of patient data
• Utilize computers in the collection, management, and reporting of patient records
• Experience working in Microsoft Word and Excel
• Ability to work with a diverse patient population
• Strong interpersonal and oral/written communication skills, including experience with direct service to individuals
• Ability to prioritize, as well as effectively re-prioritize as required by changing schedules and deadlines
• Knowledge of the organization and delivery of health and human services
• Familiarity with contraceptive health information
• Strong multi-tasking and organizational skills
• Ability to work independently, and in partnership with staff at partnering agencies
• Strong computer skills and/or willing to learn additional skills

Qualifications

MINIMUM EDUCATION:
• Graduation from high school or equivalent.
• Medical Assistant Diploma and/or National Medical Assistant Certification verifying graduation from a Medical Assistant Program or equivalent military healthcare experience.


MINIMUM EXPERIENCE:
• Completion of a medical assistant externship
• 1 year experience as a Medical Assistant in a clinical or ambulatory setting


PREFERRED EXPERIENCE:
• 2 years of experience as a Medical Assistant in a clinical or ambulatory setting


REQUIRED CERTIFICATIONS/LICENSURE:
• Current Health Care Provider Cardiopulmonary Resuscitation (CPR) certification through American Heart Association or American Red Cross. Will be required to maintain a current CPR certification during employment.
