Overview

The Program Coordinator will join a grant-funded, multi-disciplinary, and cross-organizational team focused on providing lung cancer screening and tobacco cessation services to eligible patients. The Program Coordinator provides direct support to the CUC team members, including managing and optimizing schedules for smoking cessation and lung cancer screening; contacting patients to remind them about upcoming appointments; coordinating patient transportation to appointments, if needed; ensuring appropriate documentation is provided to facilities and obtaining preauthorization from third party health insurances, if needed; completing data entry into patients' medical records; other duties as assigned. This is a grant-funded position through February 2027, with funds expected to continue.

Hybrid Work Model: This position allows for a combination of remote work at an approved off-site location and in-person work at our primary location or other sites, depending on business needs.

Responsibilities

  • Manages program service scheduling, including for smoking cessation sessions and lung cancer screening
  • Contacts patients to schedule/reschedule, provide appointment reminders, and follow-up on program services.
  • Addresses patient barriers, including coordinating transportation to appointments.
  • Follows up with external healthcare facilities on orders and referrals to ensure all required documentation has been received.
  • Ensures test results are provided back to CUC in a timely manner.
  • Completes data entry in the patient’s medical record in a timely and accurate manner.
  • Attends and participates in program and organization meetings.
  • Collaborates and communicates respectfully with patients and their families, striving to develop favorable relationships.
  • Effectively manages difficult customer services circumstances with a positive, problem-solving attitude ensuring the efficient delivery of services.
  • Maintains a commitment to improving access to health care services, particularly cancer prevention, screening, and tobacco cessation.
  • Ensures all tasks provided and associated with patient care, patient administrative processes, and related duties

    comply with all regulatory and accreditation standards including The Joint Commission, CommUnityCare Standard Operating Procedures, and CommUnityCare Policies and procedures.
  • Performs other duties as assigned.

Qualifications

  • Required: High School Diploma or Equivalent
  • Preferred: Associate's Degree - Social Work, Psychology or related field
  • Required: 2 years of experience in Social Services, Healthcare or Business Operations
  • Preferred: 1 Year of experience working with at-risk or vulnerable population
  • Preferred: Bilingual (Spanish/English)
