The Program Coordinator will join a grant-funded, multi-disciplinary, and cross-organizational team focused on providing lung cancer screening and tobacco cessation services to eligible patients. The Program Coordinator provides direct support to the CUC team members, including managing and optimizing schedules for smoking cessation and lung cancer screening; contacting patients to remind them about upcoming appointments; coordinating patient transportation to appointments, if needed; ensuring appropriate documentation is provided to facilities and obtaining preauthorization from third party health insurances, if needed; completing data entry into patients' medical records; other duties as assigned. This is a grant-funded position through February 2027, with funds expected to continue.
Hybrid Work Model: This position allows for a combination of remote work at an approved off-site location and in-person work at our primary location or other sites, depending on business needs.
