The Program Support Coordinator provides direct support to clinical operations within education and
research's staff, including the smoking cessation and navigation coordinators. Assignments could include
managing and optimizing schedules, obtaining medication and services preauthorization from third party
health insurances, data entry into patient’s medical record, staffing medication refill requests with the
prescribing staff, and other duties as assigned. The Program Support Coordinator is under the direct
supervision of the Director of Education and Research.
Essential Duties (at least 5 that are non-negotiable duties and are absolutely pertinent to successfully completing the job without
accommodations):
• Coordinate scheduling, including rescheduling and patient reminder calls
• Conduct outreach and/or follow up calls as needed
• Track and manage medication refill requests and prior authorization with the psychiatrists and/or mid-level prescriber as
needed.
• Obtain medication and services preauthorization from third party health insurances.
• Collaborate with pharmacy and prescribing provider regarding medication questions.
• Collaborate with schools and businesses that all forms requested (as approved by CommUnityCare) are completed and
returned.
• Maintain updated referral resources.
• Conduct patient chart research as it relates to medication refill requests, request paperwork (such as but not limited to:
behavioral health assessments, work/school notes, jury duty, housing, food stamp letters, etc.)
• Enter data into the patient’s medical record.
• Complete all direct and indirect care documentation in a timely manner and ensure patient records are current and
complete.
• Staff clinical issues with appropriate behavioral health staff.
• Attend all mandatory meetings and planning sessions. These may be related to quality assurance, patient care and other
related topics.
• Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with
all regulatory and accreditation standards including The Joint Commission and CommUnityCare Standard Operating
Procedures and CommUnityCare Policies and procedures.
• Complete updates on safety through the organization's computerized education system on an annual basis.
• Collaborate, communicate respectfully with patients and their families, striving to develop favorable relationships with
families.
• Be able to manage difficult customer services circumstances with a positive, problem-solving attitude ensuring the efficient
delivery of services.
• Maintain commitment to improving access to treatment and destigmatizing mental health concerns
• Perform other duties as assigned.
Knowledge of:
• Basics of behavioral health (preferred)
• Healthcare office concepts, practices, policies, and procedures
• Microsoft Office Applications (Word, Excel, Outlook)
• HIPAA and Joint Commission guidelines and regulations (preferred)
Skilled In:
• Strong interpersonal skills
• High level of skill at building relationships and providing excellent customer service
• Verbal and written communications, including telephone and email etiquette and documentation
• Bilingual (Spanish/English) preferred.
Ability to:

personal computer skills, including Microsoft Office)

MINIMUM EDUCATION: High school diploma
PREFERRED EDUCATION: Associates degree in social work, psychology, or related field
MINIMUM EXPERIENCE: 2 years of relevant work experience in social services, healthcare, or business operations
PREFERRED EXPERIENCE: 1 year of the minimum experience working with at-risk or vulnerable populations
REQUIRED COURSES/COMPLETIONS (e.g., CPR) Basic Life Support through the American Heart Association or American
Red Cross
PREFERRED: Bilingual (Spanish/English)
