Overview

The Care Model Provider Instructor is responsible for training care team members in the delivery of CommUnityCare's model of care. Under the direction of the Associate Director of Care Model Academy, they participate in curriculum development, training, and ongoing assessment of new and existing clinical staff in relation to the Gold Standard workflows established for each service line. Instructors have a strong understanding of the framework and strategies of team-based primary healthcare as practiced by high performing organizations, particularly as they relate to medical providers, and can effectively communicate these tenants. This role is high profile and impactful across all service lines of our organization by communicating effectively with providers, nursing, operations, and other leadership.

 

Instructors are responsible for teaching team members in classroom and hands-on settings, ensuring that they gain the knowledge and skills that are required to deliver our model of care at the highest level. They are responsible for sound educational practices and the academic development of each care team member, with an emphasis on medical providers. Instructors will also manage their classroom and preceptor environments and promote a positive and engaging learning experience, provide support, tutoring and other assistance as needed. The ideal candidate has skillsets in EMR/EHR systems for computer efficiencies and experience working with clinical providers to determine their skillsets and understanding of systems and processes via observation and successful application. They will seek new and innovative learning techniques while considering the learning styles of a diverse workforce. This individual will remain current on system changes and updates and effective workflows.

 

Instructors are to "model" the professional behaviors, language, and activities expected of our care team members. They will have a strong understanding of all requirements and skills that fall under the responsibility of a medical provider and will assist new or existing staff as well as problem-solve to help them grow within their roles.

Responsibilities

Essential Functions

  • Performs nursing practice in accordance to relevant policies and procedures.
  • Demonstrates a high level of understanding, knowledge, and skills in general medicine/nursing practice
  • Serve as a subject matter expert on provider workflows, and conduct site visits to provide at-the-elbow support.
  • Partner with provider in clinic setting to reinforce best practices and documentation efficiencies.
  • Work with leadership to assess the learning needs of the clinical provider staff, new and seasoned. Assess training gaps and re-educate clinical providers when deficiencies are uncovered.
  • Participate in skills development that are aligned with CUC’s established care model and in the implementation of educational programs to ensure compliance with quality standards and Gold Standard Workflow.
  • Design, deliver and maintain provider-specific training content including training plans, facilitator guides, and assessments so that they remain current and relevant, including ongoing updates and process changes.
  • Partner with the Epic training team to establish seamless workflows and provide the lived experience

  • Experience as an EPIC clinical champion and/or experience working with EPIC EHR

Qualifications

Minimum Education:

  • Associate's Degree Accredited School of Nursing

Minimum Work Experience:

  • 2 years Ambulatory or Outpatient Clinical Nursing Experience
  • 2 years Teaching experience- students, new employees, or formal education experience

Required Licenses and Certifications:

  • BCLS - Basic Life Support Upon Hire
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure Upon Hire
