STI/Adherence RN Essential Duties:
- Performs nonjudgmental verbal assessment of patient’s sexual health and STI risk factors including limited physical exam to assess STI signs and symptoms
- Orders and collects specimens for STI testing according to CDC guidelines and approved protocols
- Orders and administers treatment for known and suspected STIs according to CDC guidelines and approved protocols
- Counsels on STI signs and symptoms, prevention (including, PrEP/PEP), and partner notification.
- Follows up on STI lab results by contacting patients and scheduling treatment and/or further assessment
- Performs HIV testing for partners of established HIV positive patients who are seeking prevention services.
- Works collaboratively with Sexual Health Care Team to link partners to PrEP services.
- Manages own template/schedule to accommodate the treatment and maintenance of established HIV positive patients and established CommUnityCare patients on PrEP.
- Follows up with patients that are at risk for following out of HIV care.
- Works independently to assess ART medication adherence for established HIV patients.
- Identifies the need for further education and intervention for noncompliant HIV patients.
- Creates medication adherence care plans for HIV patients on ART.
- Collaborates with Clinical Pharmacist and primary care provider to address polypharmacy that attributes to medication nonadherence.
Floor RN Essential Duties:
- Performs Charge Nurse Duties in absence of Charge Nurse.
- Oversee assigned staff, including but not limited to, establishing work assignments and oversee flow of unit along with identifying staffing needs.
- Input for performance evaluations and serve as a support lead to the charge nurse.
- Helps identifies and participates in process improvement strategies
- Assist Charge Nurse in Care Coordination and Population Health as needed.
- Provide coaching/mentor of Staff Development of nurse and clinical team.
- Supporters nurses and clinical team to improve clinical skills and knowledge.
- Assess the physical, social and emotional needs of patients and families in clinics.
- Teach and counsel patients and families concerning health assessment and care including prevention of disease, disability, hygiene, and health maintenance.
- Participate in the identification of patient needs for referral to resources that facilitate the continuity of care in conjunction with care management team.
- Provide direct nursing care to patients and distribute medications and treatments according to accepted standards of care.
- Complete all direct and indirect care documentation in timely manner and ensure patient records are current and complete.
- Participates in daily patient care (unit) huddles, in some case ensuring that huddles are taking place along with weekly clinic staff informational huddles.
- Maintains all licensure and/or certification requirements for continuing education and best practices.
- Identifies, in collaboration with Charge Nurse, supports and participates in quality strategies in collaboration from Charge Nurse to evaluate compliance with evidence‐based guidelines, standards, and to identify opportunities to improve patient outcomes.
- Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare Standard Operating Procedures and CommUnityCare Policies and Procedures.
- Act as the site lead in the absence of the Nurse Manager Operations and/or Practice Leader.
- Daily/weekly quality assurance checklist to include delegation of tasks as appropriate to staff (Rider report, class D, McKesson, CLIA, AED, equipment checks)
Ensures state vaccine (VFC/ASN) policies are being followed. - Attends all clinic codes.
- Patient triage as needed.
- Performs periodic chart audits.
- De-escalation of crisis situations by seeking guidance of supportive personnel.
- Perform other duties as assigned.
