The Case Management Supervisor-Vulnerable Populations is responsible for collaborating with Healthcare for the Homeless, Addiction Recovery, Sexual Health, and Behavioral Health leaders on case management and social work program development and implementation in service of individuals identified as especially vulnerable or high risk. The Case Management Supervisor-Vulnerable Populations is responsible for supervising the activities of case management personnel on the homeless street medicine & mobile team, ARCH clinic, and other intensive outpatient clinic locations in providing services to patients and families. Additionally, the person in this role is responsible for applying empowerment-based prevention and intervention strategies to improve community health, making referrals to community-based resources, addressing barriers to improving well-being, offering expertise regarding psychosocial issues affecting patient care, and other case management activities.
ESSENTIAL/PRIMARY DUTIES
Essential Duties (at least 5 that are non-negotiable duties and are absolutely pertinent to successfully completing the job without accommodations):
Provide training, supervision, and support to vulnerable populations case managers
Develop & maintain weekly schedules for case management teams
Assist grant management team in developing applications for newly identified grant opportunities
Provide training and guidance to staff on processes such as applying for public benefits, the SOAR method, addiction treatment navigation, and ECHO housing assessments.
Apply empowerment-based case management strategies to reduce negative social determinants of health in high risk populations including people experiencing homelessness, those with substance use disorders, HIV, etc.
Research and provide referrals for patients to appropriate, accessible community-based resources.
Manage employee time cards and paid time off requests
Supervise bachelor or masters level social work interns & students
Assist site leadership in maintaining grant data and reporting
Assess individual needs, strengths, barriers, and readiness to change.
Address personal and environmental issues with the patient in order to maximize emotional, social, and physical wellbeing and the appropriate use of healthcare resources.
Offer the “person-in-environment” perspective, which incorporates all factors that influence a patient’s health care experience.
Provide brief crisis interventions as needed.
Provide the expertise, consultation, and education in areas relating to psychosocial issues affecting patient care for individuals experiencing homelessness, substance use disorder, or other high risk conditions.
Communicate and collaborate with clinic providers and support staff to assess patient’s needs and provide comprehensive healthcare.
Seek opportunities and implement goals for own professional development.
Complete all direct and indirect care documentation in timely manner and ensure patient records are current and complete.
Attend patient care (unit) huddles and periodic informational (clinic) huddles, meetings, patient conferences, or planning sessions. These may be related to quality assurance, patient care and other related topics within the clinic.
Attend seminars and maintain all licensure and/or certification requirements for continuing education and best practices.
Participate in quality strategies to evaluate compliance with evidence-based guidelines, standards and to identify opportunities to improve patient outcomes.
Ensure all tasks provided and associated with patient care, patient administrative processes and related duties.
Comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare
Standard Operating Procedures and CommUnityCare Policies and Procedures.
Develop and maintain favorable relationships & partnerships with co-workers and community partners.
Perform other duties as assigned.
ADDENDUM:
In addition to the above duties, team members that are actively participating in the clinical supervision process to obtain their licensed clinical social worker license will be responsible for the following additional job duties.
Assess individual needs, strengths, barriers, and readiness to change under the clinical supervision of a board certified LCSW supervisor.
Provide preliminary diagnostic impression for patients with Axis I and comorbid disorders under the clinical supervision of a board-certified LCSW supervisor.
Conduct direct delivery of mental health treatment services under the clinical supervision of a board-certified LCSW supervisor.
Knowledge/Skills/Abilities:
High level of knowledge in the area of public benefit applications (SNAP, Medicaid, Medicare, SSDI, SSI, SOAR).
Demonstrated knowledge of the current homeless services system in Austin, Texas.
High level of skill at building relationships and providing excellent customer service.
Demonstrated knowledge of addiction treatment resources.
Passion for working with individuals experiencing homelessness & substance use disorders.
High level of problem-solving skills to better serve patients and staff.
Strong attention to detail and accuracy.
Excellent verbal and written communication skills.
Demonstrated knowledge of community resources available and how to access resources for the benefit of clients who identified as belonging to an especially vulnerable population (experiencing homelessness, substance use disorders, complex mental health
diagnosis’).
Demonstrated experience conducting case management/social work services with individuals who are experiencing homelessness and other social barriers.
Demonstrated success in collaborating with other healthcare & social service organizations.
People Management/Department Management/Business Unit Management:
Collaborate with Healthcare for the Homeless and Behavioral Health leaders on case management and coordination program development and implementation.
Coordinate the development of and oversee the implementation of program goals and objectives, working in collaboration with other clinic/department managers; develop and approve schedules and standards for meeting program goals.
Assist Healthcare for the Homeless, Addiction Recovery, and Behavioral Health leaders in developing and actively monitoring outcome and process metrics for program, including patient access and clinical quality metrics; ensure timely implementation of new metrics, strategies and processes. Problem solves issues that are barriers to expected results.
Provide leadership, oversight, and direction of assigned team members. Set objectives and coach and mentor team members.
Train team members in performance of job duties, assign job tasks, and monitor task completion.
Review staff work for accuracy and completeness, evaluate job performance and counsel employees as needed.
Compile data/information for reports to review quality and productivity metrics.
REQUIRED EDUCATION: Bachelors degree in social work, psychology, sociology, or other helping profession
PREFERRED EDUCATION: Masters degree in Social Work (MSW)
REQUIRED EXPERIENCE: 2 years case management experience (paid or internship) with at least 1 year working with individuals experiencing homelessness within the last 3 years.
PREFERRED EXPERIENCE: 1 Year of Supervisory, Program Development, and/or Grant Management experience.
PREFERRED CERTIFICATIONS/LICENSURE:
1. Licensed Master Social Worker (LMSW) in the state of Texas
2. SOAR Certification
3. ECHO Coordinated Assessment Assessor Certification
4. Austin Homeless Management Information System (HMIS) License
REQUIRED COURSES/COMPLETIONS- UPON HIRE (e.g., CPR)
Current Health Care Provider Cardiopulmonary Resuscitation (CPR) certification through American Heart Association or American Red Cross. Will be required to maintain a current CPR certification during employment.
