Overview

The Minority AIDS Initiative (MAI) improves access to HIV care and health outcomes for disproportionately affected minority populations, including Black/African American and Hispanic populations. The Minority AIDS Initiative (MAI) Case Manager will provide a wide range of intensive, client centered services to persons with HIV/AIDS or at risk for HIV in a variety of settings to expedite access to medical care with the goal of HIV viral suppression, to address medical, psychosocial, and other issues that are barriers to care, and to collaborate with other programs within CommunityCare and with local service providers for the benefit of clients and patients. The MAI Case Manager also conducts outreach to community members and assists patients with linkage to care or prevention.

The MAI Case Manager will report directly to the Medical Social Services Supervisor-HIV Care at David Powell Clinic (where they will spend 80% of their time) and indirectly report to the Outreach Program Manager at Black Men’s Health Clinic (where they will spend 20% of their time). This position will be funded through the Ryan White HIV/AIDS Program Part A/Minority AIDS Initiative grant. The Minority AIDS Initiative grant is through 2/28/2026, and may be extended upon availability of funds up through 2/29/2028.

Responsibilities

Essential Functions:

  • Assess current needs of patients by conducting interviews with patients and/or family members.
  • Provide and research current and most effective community resources.
  • Address personal and environmental issues with the patient in order to maximize emotional, social, and physical wellbeing and the appropriate use of healthcare resources.
  • Maintain appropriate client files and timely documentation of services utilizing electronic health records and case management software such as EPIC, Provide Enterprise, and location‐specific software.
  • Work cooperatively with other CommUnityCare staff, inform clients of the services available through CommUnityCare and in the community, and link clients to them as appropriate.
  • Establish and maintain effective working relationships with the appropriate community resources, including those providing HIV prevention, testing and health, mental health, and social services to underserved and target populations.
  • Maintain a strong working knowledge of HIV prevention and treatment options, risk reduction techniques, chronic diseases, mental health issues, insurance and benefits, case management and other related issues through self‐study and participation in trainings, workshops, and in‐services in consultation with supervisor.
  • Engage persons from vulnerable populations, who may or may not be established in the healthcare system.

  • Strong attention to detail and accuracy. 
  • Excellent verbal and written communication skills. 
  • Quality consultation, referral, education, and other assistance, regardless of eligibility for case management, and to the extent required to optimize client success in effectively addressing their needs.
  • Ability to establish rapport and maintain effective communication with culturally diverse populations from a wide range of life circumstances and backgrounds. 
  • Demonstrated effective written and verbal communication skills, including client‐centered communication and assessment skills. 
  • Ability to effectively organize work products and files, keep clear records, and manage time well to optimize efficiency and productivity. 
  • Willingness to travel within the designated territory and willingness to work evenings and weekends as needed. 
  • Computer skills that include at minimum a working knowledge of Windows and Microsoft Office. 
  • Knowledge of and sensitivity to the needs and lived experiences of people living with HIV or those at risk of HIV and/or an eagerness to learn about this community. 
  • Bilingual (Spanish/English) preferred.

Qualifications

Minimum Education:

Bachelor's Degree (higher degree accepted) in social work, health care, or a human service discipline from an accredited college or university.

 

Minimum Experience: 

2 years offering direct client contact in a community based social service or health service agency

Licenses and Certifications:

Basic Life Support (BLS) from an approved American Heart Association or American Red Cross provider Upon Hire. Required
