The Minority AIDS Initiative (MAI) improves access to HIV care and health outcomes for disproportionately affected minority populations, including Black/African American and Hispanic populations. The Minority AIDS Initiative (MAI) Case Manager will provide a wide range of intensive, client centered services to persons with HIV/AIDS or at risk for HIV in a variety of settings to expedite access to medical care with the goal of HIV viral suppression, to address medical, psychosocial, and other issues that are barriers to care, and to collaborate with other programs within CommunityCare and with local service providers for the benefit of clients and patients. The MAI Case Manager also conducts outreach to community members and assists patients with linkage to care or prevention.
The MAI Case Manager will report directly to the Medical Social Services Supervisor-HIV Care at David Powell Clinic (where they will spend 80% of their time) and indirectly report to the Outreach Program Manager at Black Men’s Health Clinic (where they will spend 20% of their time). This position will be funded through the Ryan White HIV/AIDS Program Part A/Minority AIDS Initiative grant. The Minority AIDS Initiative grant is through 2/28/2026, and may be extended upon availability of funds up through 2/29/2028.
Essential Functions:
Knowledge, Skills and Abilities:
Minimum Education:
Bachelor's Degree (higher degree accepted) in social work, health care, or a human service discipline from an accredited college or university.
Minimum Experience:
2 years offering direct client contact in a community based social service or health service agency
Licenses and Certifications:
Basic Life Support (BLS) from an approved American Heart Association or American Red Cross provider Upon Hire. Required
