Overview

The RN Program Coordinator will support the coordination of patient care services, serving as organizational lead for our Permanent Supportive Housing (PSH) programs. In collaboration with patients, families (as defined by the patient) and staff across all disciplines and departments is responsible for planning, organizing and directing the clinical support staff assigned to PSH locations.

 

***This is an onsite position. To be considered, you must reside in the greater Austin/Travis County area***

Responsibilities

Essential Functions:

 

  • In collaboration with the Nurse Manager, supporting permanent supportive housing (PSH) efforts of assigned units by assisting with regular analysis of population health data to improve clinical care workflows to support the quality of patient care and enhance cost effectiveness.
  • Continuously monitor performance measures, PSH program deliverables and reports to ensure goals are met, documented and reported to supervisor and PSH collaboration partners.
  • Coordinate with internal and external resources, as required, for PSH program execution.
  • Serve as PSH program representative at organization-wide and community meetings, providing program data and client-level data as needed.
  • Oversee assigned staff, including but not limited to, establishing work assignments and oversee flow of unit along with identifying staff needs.
  • Input for performance evaluations and serve as a support lead to the charge nurse.
  • Performs Charge Nurse duties in absence of Charge Nurse.
  • Develop and maintain positive working relationships with internal and external partners of PSH collaboration.
  • Actively participates in Process Improvement strategies.
  • Identifies program risks through data analysis and observation; propose solutions and appropriately resolve issues.
  • Provide coaching/mentor of clinical staff, support nurses and clinical team to improve clinical skills and knowledge.
  • Assess the physical, social and emotional needs of patients and families in clinics.
  • Educate and counsel patients and families concerning health assessment and care including prevention of disease, disability, hygiene, and health maintenance.
  • Participate in the identification of patient needs for referral to resources that facilitate the continuity of care in conjunction with care management team.
  • Provide direct nursing care to patients and distribute medications and treatments according to accepted standards of care.
  • Complete all direct and indirect care documentation in a timely manner and ensure patient records are current and complete.
  • Participating in daily patient care (unit) huddles, in some case ensuring that huddles are taking place along with weekly clinic staff informational huddles, disseminating information from meetings, patient conferences, or planning sessions. These may be related to quality assurance, patient care and other related topics within the clinic.
  • Maintains all licensure and/or certification requirements for continuing education and best practices.
  • In collaboration with Charge Nurse and Nurse Manager, participates in quality strategies to evaluate compliance with evidence‐based guidelines, standards, and to identify opportunities to improve patient outcomes.
  • Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards including The Joint Commission, state vaccine (VFC/ASN) policies and CommUnityCare Standard Operating Procedures and CommUnityCare Policies and Procedures.
  • Ensure that all tasks related to the PSH program are in accordance with company policies and governmental regulations.
  • Act as the site lead in the absence of the Nurse Manager Operations and/or Practice Leader.
  • Daily/weekly quality assurance checklist to include delegation of tasks as appropriate to staff.
  • Attend all unit codes.
  • Patient triage as needed.
  • Performs periodic chart audits.
  • De‐escalation of crisis situations by seeking guidance of supportive personnel.
  • Perform other duties as assigned.

  • Knowledge of permanent supportive housing program, the collaboration with external partners, its requirements and guidelines.
  • High degree of knowledge and competency in the practice of nursing and documentation requirements.
  • Ability to work under pressure, prioritize tasks, manage competing objectives in an ambitious fastpaced environment to achieve results and meet deadlines.
  • Excellent verbal and written communication skills with ability to communicate effectively with various audiences.
  • Ability to work independently and in a team environment, with demonstrated success in directing staff to achieve a common goal.
  • High level of critical thinking/problem solving skills; identifying problems, evaluating alternatives and recommending/implementing solutions.
  • Ensure all actions, work performance, personal conduct and communications always represent CommUnityCare in a highly professional manner.

 

Qualifications

Minimum Qualifications:

 

Education:

Associates Degree (higher degree accepted) in Nursing

 

Work Experience:

At least One (1) year in an administrative nursing position required 

 

Licenses and Certifications:

Unrestricted (RN) Registered Nurse in the State of Texas

Basic Life Support Certification-current for Healthcare Provider from the American Heart Association
