Overview

The Director of Adult Health (DAH) oversees the Adult Health Division of the Clinical Services Department. The DAH establishes the clinical standards of care, the infrastructure for performance improvement, and framework to support optimal wellness of the providers. Working closely with the CMO, DAH creates functional and specific objectives for the Adult Health Division in pursuit of CommUnityCare’s strategic goals. This includes establishing and sustaining workflows, protocols and goals that promote health equity, clinical excellence, workforce wellbeing, exceptional patient experience and resourcefulness. In particular, the DAH leads the development and reinforcement of CommUnityCare’s (CUC) team-based care model, inclusive of guiding the design and strategy of CUC’s electronic health record workflows pertinent to the Division. In collaboration with data analytics, operations and nursing leadership, the DAH establishes core clinical and operational performance measures, overseeing the data-driven, continuous quality improvement of population health and primary care initiatives and programs for adults. Through mentorship and supervision, the DAH drives delivery of efficient care that is clinically competent, culturally sensitive, patient centered, and meets or exceeds performance goals. The DAH functions as an integral member of CUC clinical leadership team and demonstrates a keen understanding of the health care industry in general and specifically of federally qualified health center operations.

Responsibilities

Essential Duties (at least 5 that are non-negotiable duties and are absolutely pertinent to successfully completing the job without accommodations):

 

·         Establish clinical standards of care for Adult Health Services, inclusive of care pathways, protocols, and evidence-based and best practices, to establish and maintain the highest standards of quality, performance, and cost-effectiveness.

·         Establish, update, and meet or exceed key clinical, operational, and financial goals and objectives for the Adult Health service line.

·         Advise CMO related to strategic direction of adult health services.

·         Develop and institute performance improvement metrics and processes focused on a high performing, high quality service delivery model.

·         Lead clinical and operational health equity and quality improvement initiatives for the service line in promotion of optimal outcomes, including but not limited to continuity of care and patient retention.

·         Track and oversee quality team-based care delivery, inclusive of multi-disciplinary care coordination.

·         Establish and promote gold-standard clinical workflows in pursuance of high performance and optimal outcomes.

·         Ensure standardization of best practices throughout the Adult Health Services division, incorporating regulatory requirements at the local, state, and national level utilizing benchmarking data and analysis from applicable sources.

·         Promote collaboration with appropriate community partners to augment CommUnityCare’s Adult Health Services and advance the organizational mission and vision.

·         Partner with clinical services leadership to support a comprehensive, effective Wellness Program.

·         In collaboration with clinical operations and nursing, promotes excellence in medical education among all learners.

·         Exemplify service delivery excellence

·         Serve as active participant of clinical leadership committees and meetings as delegated by the CMO.

·         Perform comprehensive assessment of patients using observation, inspection, auscultation, palpation and percussion.

·         Order or execute various tests, analyses and diagnostic images to provide information on patient’s condition.

·         Diagnose and treat diseases, disorders and injuries based on patient’s medical history and results of physical assessment and refer patients to medical specialist or other practitioner for specialized treatment.

·         Prescribe medications and therapies for treatment, preventive care, and contraception (if appropriate).   

·         Direct professional and ancillary health care staff during treatment of patients.

·         Respond to inquiries from staff and patients regarding preventative health and treatments.

·         Compile patient medical data including health history, treatment plans, and results of physical examination, etc. and record in electronic medical record.

·         Report required information including deaths and outbreak of contagious diseases to governmental authorities.

·         Plan, promote, demonstrate, recommend, and coordinate high standards of medical practice.

·         Performs up to 16 hours of administrative time per week.

·         Perform other duties as assigned.

·         Demonstrated understanding and/or experience working in an integrated delivery environment.

 
People Management/Department Management/Business Unit Management:

 

·         Oversees performance and development of Associate Directors of Adult Health and Child/Adolescent Health, and indirectly the Clinical Leads at all Health Centers.

·         Assumes full responsibility for primary care patients receiving care within the designated health centers.

·         Works in tandem with the Health Center’s practice and clinical leaders to ensure a collaborative team oriented environment.

·         Leads and motivates the medical providers under his/her supervision; uses effective medical and people performance management processes.

·         Supports, leads and models the vision, mission and values of CUC.

·         Serves as the champion for Value Based Care initiatives and other initiatives that may impact the operations of CUC and/or the quality of patient care.

·         Performs objective annual evaluations of all direct reports with the input from other departmental leadership.

·         Supervises the Clinical Nutrition Manager, and works in close collaboration with the Director of Behavioral Health and Director of Pharmacy to empower the Behavioral Health Manager and Associate Director of Clinical Pharmacy, respectively, to lead their departments as high performing collaborative care team members within the primary care model.

Qualifications

MINIMUM EDUCATION:  M.D. or D.O. from an accredited educational institution

MINIMUM EXPERIENCE: 2 years of clinical experience

REQUIRED CERTIFICATIONS/LICENSURE:

1.       Unrestricted License to practice medicine in the state of Texas.

2.       Board Certification in Family Medicine, Internal Medicine, or Pediatrics or passage of Board Certification examination within one year of employment. 

3.       Current Drug Enforcement Agency (DEA) registration for the purpose of writing prescriptions.

 

REQUIRED COURSES/COMPLETIONS (e.g., CPR):  Current Healthcare Provider Cardiopulmonary Resuscitation (CPR) course completion card.  Basic Life Support Certification for Healthcare Providers.
